The forms and the article

: The possessive adjective indicates who something or someone belongs to (il mio libro, la tua casa). Like all adjectives, it agrees in gender and number with the noun that follows, not with the owner.

Regola d'oro: possessivo + articolo
In Italian the possessive almost always goes together with the article: il mio libro, la mia penna, i miei amici, le mie idee. This is a big difference from English (my book) and Polish (moja książka).
With family nouns in the singular the article drops: mia madre, tuo padre, sua sorella. In the plural, however, the article comes back: le mie sorelle, i tuoi fratelli.
Eccezione «loro»
The exception to the exception: with «loro» the article always stays, even with singular relatives (la loro madre), and loro is invariable: it never changes form.
la loro madreil loro padrei loro figli
suo = di lui / di lei
Note: il suo / la sua mean both «his» and «her». It is the context that makes it clear: Marco legge il suo libro (Marco's); Anna legge il suo libro (Anna's).
il suo cane (di lui o di lei)la sua idea
